Can't say as I've ever seen this... but i'm thinking the AEM management not being properly tuned + the new cat-back exhaust had something to do with this

Quick guess:
Poor engine tune resulted in a high quantity of unburnt gasoline entering the catalyst where it was then ignited by the heat of the catalyst area. Gasoline exploded, as it tends to do, and loosened up the inside of catalyst which shot out the back.

why didn't he turn the car off after the exhaust started shooting bits and pieces? ... dumbass

That was really dumb of him to continue destroying his cats... they probably cost a fair amount to replace. As someone else said tho, probably a case of it running way rich and raw fuel backfiring thru the cats.

once a single chunk is blown out they will continue to fall apart no matter what you do. With all the work he is going to do to the car I doubt he was going to keep any cats on the car and if he was they would have probably been aftermarket high flow ones.
